The forms are taken from information presented in Appendix 1 of Dr. Neufeld's book, Kidshape - A Practical Prescription for Helping your Child. These charts and worksheets can be used by your family to begin implementing health changes. The book explains how to use them in more details and can be purchased at amazon.com.

INSTRUCTIONS TO DOWNLOAD MATERIALS

1) Right Click on the link above labeled 'Download Journal and Workshop Materials.' Select 'Save Target As' and save it on your harddrive.

2) Once it's finished downloading you will get a pop up window 'Download Complete' and Select the 'Open' button.

PRINTING INDIVIDUAL PAGES

1) To print individual pages go to your menu. Select Files > Print.

2) Now on your print window you will see a section named Print Range. You can select the Pages that you want to print. Per say if you want to print page 216 - 218. Then you enter from: 216 to: 218. Remember that page 216 is the same as page 1, page 217 is the same as page 2, page 218 is the same as page 3, so and so forth. If you enter from: 1 to 3 will be the same as printing 216 - 218. If you have any trouble printing individual pages contact your local administrator.
