| | With its outstanding success rate and ease of implementation, KidShape® has been approached by health professionals across the country seeking an effective treatment program for childhood obesity. Licensing is simple and is designed solely to ensure the integrity of the program. Interested healthcare providers and facilities are required to sign an agreement committing to adhere to the program implementation procedures. Training is provided and the KidShape® staff stands ready to assist throughout the implementation phase. | | Pilot Test To maintain the integrity of KidShape®, each program will be implemented once as a pilot test. Each hosting facility must participate in the KidShape® Foundation two day on-site training program. The hosting facility will collect data and report back to the KidShape® Foundation. The program curriculum and workbooks will be adjusted based on the outcome data and information collected from participating families and staff. The pilot test covers the licensing fee for one site and is valid for one year and includes the required on-site training by a KidShape® Foundation Instructor. Travel and per diem for KidShape® instructor is additional.
Training Program ONE-DAY ON-SITE TRAINING (approximately 7 hours) provides each health care provider a comprehensive understanding of how to implement the KidShape® program. The program manual and program implementation is reviewed week by week. This training allows the provider to know what to expect and how to implement each of the eight-weekly classes of the KidShape® program. ONE-DAY INTAKE & COMMUNITY OUTREACH TRAINING (approximately 6 hours) provides comprehensive training to the facility intake staff, who will be enrolling families into the program. The intake process is reviewed from the point of the initial referral to obtaining appropriate authorizations. KidShape® Foundation has developed Access database to track each family's demographics and participation in the program. We review how to enter data and generate necessary reports to provide useful information about program outcome data. The KidShape® database will also allow you to generate letters to referring health care providers about the status of their patients. A comprehensive individualized community-outreach plan will also be developed to assist the hosting facility with generating on-going referrals. Licensing

- Ability to purchase KidShape® workbooks, program materials, and program manuals
- Patient Data Base Template for tracking participant demographics and outcome data
- Hosting facility will be advertised on the KidShape® Foundation website with a link to the hosting facility's website, if available.
- Letter templates, connected to data base, to provide referring healthcare provider feedback on patient's progress
